健康政策和要求

参加伊利诺伊大学需要强制健康表格。 The Mandated Health Forms for new students can be found on the 登记清单 and are listed below:

体检表和免疫记录必须由医疗保健提供者填写并签名。 必要的医疗信息表必须由学生填写并由家长签署（如适用）。

All documentation must be uploaded to the Chesley Health and Wellness Center app located on 单点登录门户（SSO） by 7月1日 for the Fall semester and 12月1日 for the Spring semester. 所有项目的完成将加快你在新生培训中的进步。 未能在上课第10天之前提供规定的健康表格的学生将导致注册帐户被冻结。
